Extrinsic Cleaning Failure

A cleaning failure that is not necessarily due to the process itself

A failure not attributable to the cleaning process but rather an uncontrollable external factor leading to the cleaning process not being performed as intended. (e.g., power failure to the building or the equipment, or fire alarms require personnel to evacuate, and equipment could not advance during a hold step)
